Patient preferences for therapy, medication, or both, are included in the treatment plan, and any referrals necessary will be made. Medication, if necessary and agreed upon, will be called in to the patients preferred pharmacy and any remaining questions or concerns will be addressed. A follow-up visit to assess ongoing effectiveness of the treatment plan will be made. At consecutive appointments, any necessary changes to the treatment plan will be made. However, I am available for concerns, questions, or needs that may arise prior to follow-up visits when needed.
